Overview
We are seeking a compassionate and skilled Home Health Physical Therapist to join our Phoenix team. In this role, you will deliver personalized, evidence-based therapy services that help patients regain mobility, independence, and quality of life in the comfort of their homes. As a key member of the interdisciplinary care team, you'll collaborate with nurses, social workers, and other clinicians to ensure coordinated, patient-centered care that achieves meaningful outcomes.


Key Responsibilities

Patient Care Excellence

  • Provide individualized physical therapy interventions that improve mobility, strength, and overall functionality.
  • Incorporate evidence-based practices to optimize patient recovery and care outcomes.

Operational Performance

  • Develop and adjust therapy care plans to align with patient goals and evolving needs.
  • Monitor progress, track outcomes, and document thoroughly in the electronic health record (EHR).
  • Manage caseload efficiently while maintaining high standards of care.

Collaboration & Support

  • Work closely with interdisciplinary team members to ensure holistic care delivery.
  • Mentor therapy assistants or junior clinicians, fostering professional growth.

Patient Engagement & Outcomes

  • Build trust with patients and families by providing compassionate care and clear education.
  • Promote long-term improvements in patient health while reducing hospital readmissions.

Qualifications

  • Bachelors or Doctorate in Physical Therapy (DPT) from an accredited program.
  • Active state licensure as a Physical Therapist.
  • Minimum 2 years of physical therapy experience, preferably in a home health setting.
  • Strong clinical knowledge of therapy practices, regulations, and compliance requirements.
  • Excellent communication, problem-solving, and time management skills.
  • Proficiency with EHR systems and telehealth tools.
  • Familiarity with Medicare and Medicaid home health guidelines.

Work Environment

  • Primarily in patients' homes within the Phoenix service area; occasional office visits for meetings or training.
  • Fast-paced, patient-focused environment requiring adaptability and organization.
