Job Title: RN Hospice Case Manager
Location: North Kansas City, Missouri
Salary: $80,000-$85,000
Shift: Full-time, weekday schedule

Job Overview:
We are seeking a compassionate and experienced Registered Nurse to join our Home Health and Hospice team as a Hospice RN Case Manager. This full-time role focuses on managing hospice patients in the home and senior living settings, providing coordinated, patient-centered care throughout the end-of-life journey. The Hospice Case Manager oversees assigned patient caseloads, collaborates with interdisciplinary teams, and ensures continuity of care. This position offers stability and longevity with a well-established hospice organization and a supportive, mission-driven culture.

Coverage Details:

  • Coverage Area: North Kansas City, including Liberty, Platte City, Excelsior Springs, Blue Springs, and Independence
  • Average Daily Mileage: Approximately 50 to 100 miles per day, typically 50–70 miles
  • Mileage Reimbursement: $0.45 per mile, excluding mileage to and from home

Key Responsibilities:

  • Manage and coordinate hospice patient caseloads in home and senior living settings
  • Conduct comprehensive patient assessments and ongoing evaluations
  • Develop, implement, and update individualized hospice plans of care
  • Provide skilled nursing care and symptom management for hospice patients
  • Collaborate with physicians, social workers, chaplains, and other interdisciplinary team members
  • Educate patients and families on disease progression, comfort measures, and care plans
  • Maintain accurate and timely documentation using electronic charting systems
  • Travel between patient locations throughout the service area as part of daily responsibilities
  • Ensure compliance with hospice regulations, standards of care, and organizational policies

Required Qualifications:

  • Active Registered Nurse (RN) license in Missouri and/or Kansas, as applicable
  • Prior hospice experience required
  • U.S. citizenship or valid green card
  • Strong clinical assessment, case management, and time management skills
  • Ability to work independently in a home-based care environment
  • Excellent communication and interpersonal skills
