Position: Integrated Care Nurse Practitioner (Hybrid)
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Hybrid Role: 1 day/week telehealth (remote) + 4 days in patients' homes
(Occasionally 2 remote days, depending on the week)
No Weekends | No On-Call
Compensation: Base salary + 15% quarterly bonus

Overview
We are seeking experienced Nurse Practitioners and Physician Assistants to join a home-based care team serving patients with chronic and complex conditions, including kidney disease. Youll work as part of an integrated care team alongside a Registered Nurse and Social Worker, delivering personalized, evidence-based care through in-home visits and limited telehealth.

Key Responsibilities

  • Conduct comprehensive assessments and annual wellness visits (home and virtual)
  • Develop, implement, and adjust individualized treatment plans
  • Prescribe medications and order diagnostic testing
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Educate patients and families on disease management and preventive care
  • Maintain timely, accurate clinical documentation
  • Participate in integrated care team meetings
  • Follow up and evaluate patient progress to improve outcomes

Requirements

  • Active, unrestricted NP or PA license
  • Board certification (NP: ANCC/AANP | PA: NCCPA)
  • Active DEA license
  • 2+ years of direct patient care experience
  • Experience with annual wellness visits (home or virtual preferred)
  • Ability to practice autonomously
  • Valid driver's license, reliable transportation, and insurance
  • Managed Care / IPA / Health Plan experience preferred
