Overview
We are seeking a dedicated Home Health Physical Therapist to provide personalized, evidence-based therapy to patients in their homes. This role focuses on improving mobility, independence, and overall quality of life, while collaborating with an interdisciplinary team to ensure comprehensive, patient-centered care. The ideal candidate will have strong clinical skills, a patient-first mindset, and the ability to manage a diverse caseload efficiently.


Key Responsibilities

Patient Care Excellence

  • Evaluate patients and develop individualized treatment plans focused on mobility, independence, and functional improvement.
  • Deliver high-quality, evidence-based therapy interventions in the home environment.
  • Educate patients and caregivers on exercises and strategies to promote recovery and prevent injury.

Operational & Clinical Performance

  • Adjust therapy plans as needed based on patient progress and evolving needs.
  • Maintain thorough and timely documentation in the electronic health record (EHR) to ensure compliance and care continuity.
  • Monitor outcomes and track progress toward established goals.

Collaboration & Teamwork

  • Work closely with nurses, social workers, and other therapists to provide coordinated, holistic care.
  • Participate in interdisciplinary team meetings to review patient progress and align care plans.
  • Mentor therapy assistants or junior clinicians as needed.


Qualifications

  • Bachelors or Doctorate degree in Physical Therapy (DPT) from an accredited program.
  • Active state licensure as a Physical Therapist.
  • Minimum 2 years of physical therapy experience, preferably in home health or a similar setting.
  • Strong knowledge of evidence-based therapy practices and home health regulations.
  • Excellent communication, problem-solving, and time management skills.
  • Proficiency in EHR systems and telehealth tools.
  • Familiarity with Medicare, Medicaid, and state-specific home health guidelines.


Work Environment

  • Primarily provides care in patients homes, with occasional office visits for meetings or training.
  • Regular travel within the assigned service area is required.
  • Dynamic, patient-centered environment requiring adaptability, organizational skills, and the ability to balance multiple priorities.
