What You'll Do:

• Provide skilled nursing care to patients in a home health setting

• Monitor patient condition and communicate updates to the care team

• Coordinate with physicians, families, and support services to ensure optimal outcomes

• Maintain accurate documentation and comply with agency guidelines

• Travel locally to patient homes throughout Western PA

Required Qualifications:

• Associate degree or higher in Nursing

• Active RN license in Pennsylvania (PA)

• 1+ year of RN experience (home health preferred)

• Valid Drivers License, reliable transportation, and auto insurance

• Certification in one of the following:

  • AHA BLS, AHA ACLS, or CPR (support provided if needed)

• Must confirm:

  • You have not applied to or worked for AHN Health Care @ Home in the past 6 months
  • You have spoken with a an additional recruiter by phone
  • You are comfortable with required travel
