Position: Registered Nurse - Long Term Care (LTC)

Job Summary:

We are currently seeking a Registered Nurse (RN) with experience in Long Term Care (LTC) to join our team on a 3-month contract basis. The RN will be responsible for providing direct patient care and ensuring the overall well-being of residents in a long-term care facility.

Responsibilities:

- Conduct initial and ongoing assessments of residents' health status

- Develop and implement individualized care plans for each resident

- Administer medications and treatments as prescribed by the physician

- Monitor and document residents' progress and response to treatments

- Collaborate with interdisciplinary team members to provide holistic care

- Educate residents and their families on health management and disease prevention

- Ensure compliance with all state and federal regulations and facility policies

- Maintain accurate and timely documentation of all patient care activities

- Communicate effectively with physicians, healthcare team members, and family members

Qualifications:

- Valid RN license in the state of practice (VT)

- Minimum of 1 year of experience in long-term care

- BLS/CPR certification

- Excellent clinical and critical thinking skills

- Ability to work independently and as part of a team

- Strong communication and interpersonal skills

- Knowledge of state and federal regulations for long-term care facilities

- Ability to multitask and prioritize tasks effectively

- Must be able to work 12-hour shifts and weekends as needed
