About the job Primary Care Nurse Practitioner (Long-Term Care/Geriatric Focus)
Prosperity Workforce Solutions is seeking a Nurse Practitioner to provide comprehensive primary care services within a community clinic that supports Long-Term Care (LTC) and geriatric populations.
This role focuses on delivering primary care services to older adults and LTC residents, many of whom present with complex chronic conditions and mental health needs. The Nurse Practitioner will work collaboratively within an inter-professional healthcare team, providing assessment, treatment, and ongoing management for a broad range of medical conditions.
The LTC home operates within a community clinic setting, allowing the Nurse Practitioner to deliver integrated primary care services with both geriatric and mental health components.
The program includes a recently implemented Mobile Health Clinic (Healthcare on Wheels), meaning the Nurse Practitioner will support care delivery not only within community clinic locations but also directly within the community via a mobile van and other health hubs.
This is a unionized position with a structured salary range and annual step increases each November.

Hourly Rate:
$52.83 - $74.83
Key Responsibilities

Provide assessment, diagnosis, treatment, and follow-up care for acute and chronic illnesses.

Deliver primary care services to LTC residents and geriatric patients.

Manage complex chronic conditions and age-related health concerns.

Provide care for patients presenting with mental health concerns commonly associated with geriatric populations.

Conduct comprehensive health assessments and care planning.

Develop and implement individualized, patient-centered treatment plans.

Provide medication management and monitoring.

Support patients and families through education, care planning, and treatment management.

Participate in peer/chart audits and quality improvement initiatives.

Work collaboratively with physicians, nurses, and allied health professionals within an inter-professional care team.

Provide telephone consultation and follow-up care as required.
Qualifications

Master's prepared Nurse Practitioner with Primary Health Care Certificate

Registration in good standing with the College of Nurses of Ontario – Extended Class

Member of NPAO with professional liability coverage

3+ years of Nurse Practitioner practice experience

Experience in primary care strongly preferred

Experience in long-term care, geriatrics, or senior care settings preferred

Experience working with mental health and addiction populations considered an asset

Demonstrated ability to work independently with strong clinical decision-making skills

Excellent communication and interpersonal skills
