General Primary Care Nurse Practitioner 

Part-Time | 1-Year Contract (7 hours/week)

Prosperity Workforce Solutions is seeking a Primary Care Nurse Practitioner on behalf of a community health organization serving diverse populations in Scarborough.

This role focuses on delivering comprehensive, family-centered primary care to unattached patients, with an emphasis on health promotion, disease prevention, and equitable access to care.

Key Responsibilities

• Provide comprehensive primary care across the lifespan within NP scope of practice
• Assess, diagnose, and manage acute and chronic conditions
• Deliver preventive care, including routine and catch-up immunizations
• Develop and implement patient-centered treatment plans
• Collaborate within an interdisciplinary team and participate in case conferences
• Support marginalized populations, including those with complex health, mental health, and addiction needs
• Participate in health promotion initiatives and community outreach
• Maintain accurate clinical documentation and ensure confidentiality
• Contribute to quality improvement initiatives and clinical audits
• Participate in on-call rotation and occasional home visits as required

Qualifications

• Master's degree in Nursing with Primary Health Care Nurse Practitioner certification
• Registered in good standing with the College of Nurses of Ontario (Extended Class)
• 3–5 years of NP experience, preferably in primary care or community health
• Strong clinical assessment and independent decision-making skills
• Experience working with diverse and underserved populations
• Current CPR certification
• Valid driver's license and access to a vehicle

Schedule & Compensation

• Part-time: 7 hours per week
• Contract: 1 year
• Compensation: $52.83 – $74.83/hour
• May include evenings, weekends, and on-call responsibilities
