Compensation: $72.65/hour

  • Opportunity for hourly rate increase after 3 months

Schedule:

  • Full-Time: 35 hours/week (Monday to Friday, 9:00 AM 5:00 PM)
  • Part-Time: 0.5 FTE considered - eligible for full-time benefits 
  • Occasional evening or weekend work may be required
  • Occasional on-call

Note: Travel between communities within Scarborough can be expected

Responsibilities

  • Provide comprehensive, evidence-based care within NP scope of practice
  • Conduct client assessments, develop and implement care plans
  • Monitor and follow up with patients managing acute and chronic illnesses
  • Conduct interviews, physical exams, and health promotion interventions
  • Collaborate within an inter-professional team model
  • Adhere to College of Nurses of Ontario (CNO) standards
  • Participate in peer reviews, case conferences, and on-call schedules as needed
  • Deliver services to diverse, often marginalized populations, including those with chronic illness, mental health conditions, and addictions
  • Precept NP and medical students and participate in ongoing professional development
  • Support patient self-management and provide community education sessions as needed
  • Maintain relationships with other healthcare providers, academic institutions, and community organizations
  • Provide phone support and follow-up where necessary

Requirements

  • Masters Degree in Nursing with Nurse Practitioner Primary Health Care Certificate
  • Registered with the College of Nurses of Ontario (Extended Class), in good standing
  • Member of NPAO with valid professional liability insurance
  • Current HCP CPR certification
  • Willingness to participate in professional associations (e.g.,RNAO)
  • Valid Ontario driver's license and access to a personal vehicle required
  • Legal right to work in Canada
  • Vulnerable Sector Screening

Assets

  • Experience as a Nurse Practitioner in a community health setting (strongly preferred)
  • Background in program development
  • Pediatric experience
