Prosperity Workforce Solutions is seeking a Nurse Practitioner to provide comprehensive care to patients with acute, chronic, mental health, and addiction-related medical conditions in a community health setting serving the Scarborough community. The Nurse Practitioner will deliver primary care services focused on preventive care, health promotion, and health maintenance, while supporting individuals facing complex health and social challenges.

This role works within an inter-professional model of care, collaborating with healthcare providers and community partners to deliver coordinated, high-quality care to marginalized populations. The Nurse Practitioner will also contribute to patient education, case management, and care coordination, while serving as a clinical resource to other healthcare team members.

The program includes a recently implemented Mobile Health Clinic (Healthcare on Wheels), meaning the Nurse Practitioner will support care delivery not only within community clinic locations but also directly within the community via a mobile van and other health hubs.

This is a unionized position with a structured salary range and annual step increases each November.

Employment Type:
Full-time | Monday–Friday | 37.5 hours per week

Hourly Rate:
$52.83 - $74.83

Key Responsibilities

  • Provide assessment, diagnosis, intervention, monitoring, and follow-up for acute and chronic illnesses.

  • Conduct comprehensive client interviews and physical examinations, and develop client-centered treatment plans.

  • Integrate evidence-based health promotion and preventive care practices.

  • Deliver care to individuals experiencing mental health challenges, substance use concerns, and complex social determinants of health.

  • Participate in case management and care coordination to ensure continuity of care for vulnerable populations.

  • Administer primary care services including injections, Pap tests, and other routine clinical procedures.

  • Provide patient education, treatment instructions, and support for medication management and side effect monitoring.

  • Provide telephone support and follow-up care as required.

  • Participate in peer and chart audit reviews as required.

  • Support community education initiatives when scheduled.

  • Work collaboratively within an inter-professional team environment.

  • Participate in professional development opportunities and precept Nurse Practitioner or medical students.

  • Deliver care across multiple community locations, including:

    • Community health clinics

    • Community hubs

    • Mobile health clinic services

  • Demonstrate a strong commitment to serving marginalized and underserved populations.

Qualifications

  • Master's prepared Nurse Practitioner with Primary Health Care Certificate

  • Registration in good standing with the College of Nurses of Ontario (CNO) – Extended Class

  • Member of NPAO with additional professional liability coverage

  • 3+ years of practice as a Nurse Practitioner, utilizing advanced nursing assessment skills within an inter-professional care model

  • Experience working with mental health and addiction populations strongly preferred

  • Experience supporting patients impacted by social determinants of health

  • Case management experience preferred

  • Community health experience strongly preferred

  • Experience with pediatric populations considered an asset

  • Demonstrated ability to work independently with a high level of clinical judgment and organization

  • Strong interpersonal, communication, and conflict resolution skills

  • Candidates with relevant placements, practicums, or consolidations working with similar populations will also be considered

  • A demonstrated passion for community health and working with marginalized populations
