Employment Type: Full-Time (35 hours/week) / Part-Time (21 hours/week)

Key Responsibilities
- Deliver comprehensive, family-centered primary health care within the full NP scope of practice.
- Assess, diagnose, and manage acute and chronic conditions for clients across the lifespan.
- Conduct client interviews, physical exams, and develop evidence-based treatment plans.
- Administer immunizations according to Ontarios publicly funded immunization schedule.
- Maintain accurate and confidential client health records in compliance with CNO standards.
- Collaborate with physicians and allied health professionals as part of an interdisciplinary team.
- Participate in case conferences, peer reviews, and health promotion/outreach initiatives.
- Support continuous quality improvement and best practice initiatives.
Qualifications
- Masters Degree in Nursing with Primary Health Care Nurse Practitioner (NP-PHC) certification.
- Current registration with the College of Nurses of Ontario (Extended Class) in good standing.
- Minimum 3 years of NP experience in primary care, community health, or similar settings (CHC, FHT, NP-led clinic).

- Current CPR (HCP) certification.
- Strong assessment, clinical reasoning, and documentation skills.
- Excellent interpersonal and communication skills; able to work collaboratively within a team.
- Experience serving culturally, socially, and economically diverse populations.
- Familiarity with EMR systems.
- Valid Ontario drivers license and access to a vehicle considered an asset.
