Key Responsibilities

  • Deliver comprehensive, family-centered primary health care within the full NP scope of practice.
  • Assess, diagnose, and manage acute and chronic conditions for clients across the lifespan.
  • Conduct client interviews, physical exams, and develop evidence-based treatment plans.
  • Administer immunizations according to Ontarios publicly funded immunization schedule.
  • Maintain accurate and confidential client health records in compliance with CNO standards.
  • Collaborate with physicians and allied health professionals as part of an interdisciplinary team.
  • Participate in case conferences, peer reviews, and health promotion/outreach initiatives.
  • Support continuous quality improvement and best practice initiatives.

Qualifications

  • Masters Degree in Nursing with Primary Health Care Nurse Practitioner (NP-PHC) certification.
  • Current registration with the College of Nurses of Ontario (Extended Class) in good standing.
  • Minimum 3 years of NP experience in primary care, community health, or similar settings (CHC, FHT, NP-led clinic).

  • Current CPR (HCP) certification.
  • Strong assessment, clinical reasoning, and documentation skills.
  • Excellent interpersonal and communication skills; able to work collaboratively within a team.
  • Experience serving culturally, socially, and economically diverse populations.
  • Familiarity with EMR systems.
  • Valid Ontario drivers license and access to a vehicle considered an asset.
