Licensed Practical Nurse (LPN) – Home Care

Location: West Philadelphia, PA
Schedule: Contract, 40 hours weekly, 8am – 430pm

Position Overview

The Licensed Practical Nurse delivers high-quality, patient-centered nursing care to clients in their homes. Working independently and as part of an interdisciplinary team, the LPN supports individuals with acute, chronic, or post-operative needs, promoting health, independence, and quality of life.

Key Responsibilities

  • Provide direct nursing care in patients' homes in accordance with individualized care plans
  • Administer medications, injections, and treatments as prescribed
  • Perform wound care, dressing changes, and monitor healing progress
  • Assess patient condition and report changes to the RN or care team
  • Assist with activities of daily living (ADLs) such as bathing, grooming, and mobility support
  • Educate patients and families on disease management, medications, and home safety
  • Maintain accurate and timely clinical documentation
  • Coordinate care with RNs, physicians, and other healthcare professionals
  • Ensure infection prevention and control practices are followed in the home setting
  • Travel between patient homes within the community

Qualifications

  • Active LPN license in Pennsylvania
  • Current BLS (Basic Life Support) certification
  • Valid driver's license and reliable vehicle (required for community visits)
  • Minimum 2 years of nursing experience preferred
  • Previous home care or community health experience is an asset
  • Wound care experience
