Registered Nurse - Home Care

Title: Registered Nurse (RN) Case Manager 

Location: Bala Cynwyd, PA (local contract)

Time type: 8:00am-4:30pm (8:00 AM - 4:30 PM)

Contract duration: 13 weeks

Job description:

• coordinate and oversees patient care across the healthcare continuum.
• assess patient needs, develop care plans, collaborate with providers, and ensure efficient, high-quality outcomes while managing resources and supporting patients and families.

Required qualifications : 

• 2 years of home health experience
• Must have case management (OASIS experience to be considered)

Preferred qualifications :

• Wound care experience
