Primary Care Nurse Practitioner – Youth Wellness (YWHO)

Prosperity Workforce Solutions is hiring a Primary Care Nurse Practitioner on behalf of a community health organization in partnership with a youth-focused mental health agency.

This role is embedded within the Youth Wellness Hubs Ontario (YWHO) model, providing low-barrier, youth-centered care for individuals aged 12–25, with a strong focus on mental health, substance use, and holistic primary care.

Key Responsibilities

• Provide episodic primary care to youth (ages 12–25), including assessment, diagnosis, and treatment
• Manage acute concerns and stable chronic conditions
• Integrate mental health and substance use care into treatment planning
• Order and interpret diagnostics and prescribe medications within scope
• Apply trauma-informed, harm reduction, and culturally responsive care approaches
• Collaborate with an interdisciplinary team (e.g., therapists, peer support, case workers)
• Participate in complex care planning and case discussions
• Support health education and system navigation for youth and families
• Contribute to quality improvement, program development, and clinical initiatives
• Provide mentorship and clinical leadership within the team

Qualifications

• Master's degree in Nursing with Primary Health Care Nurse Practitioner certification
• Registered in good standing with the College of Nurses of Ontario (Extended Class)
• Experience working with youth, mental health, and/or substance use populations preferred
• Strong knowledge of social determinants of health and community resources
• Excellent clinical judgment, collaboration, and communication skills
• Commitment to equity, diversity, and inclusive care practices

Schedule & Compensation

• Part-time: 21 hours per week
• Permanent position
• Compensation: $52.83 – $74.83/hour
• Includes HOOPP pension, extended health & dental benefits
• Some evenings and occasional weekends required
