Schedule: Primarily weekday daytime with flexibility to meet patient needs

Travel: Frequent local travel within the Greater Sacramento service area (driving required)

Position Overview

The Occupational Therapist (OT) provides skilled occupational therapy services to patients in their homes, helping them maximize functional independence and safety in daily living. In alignment with the agencys mission to deliver high-quality home health services regardless of socioeconomic status, the OT delivers patient-centered care that brings the care patients deserve without going far.

The OT collaborates with nursing and the interdisciplinary team to deliver effective plans of care, support positive patient outcomes, and ensure timely documentation in Finale EMR.

This nurse practitioner–owned and operated home health agency was established in 2017 to help patients recover, improve function, and live independently and well at home. The agency delivers compassionate, high-quality, patient-centered services across the Greater Sacramento region.

Patient satisfaction is a core goal, supported by after-hours clinical triage for patient concerns. Therapy clinicians are not scheduled for on-call or weekend rotations.

Work Setting & Schedule (Home Health)

Work Setting: Home Health (field-based; patient visits in the home/community)

Schedule: Full-time or near full-time based on patient census; primarily weekday daytime with flexibility to meet patient needs

Travel: Frequent local travel required within the Greater Sacramento service area

Service Area: Greater Sacramento region (Sacramento County and surrounding communities)

Productivity/Caseload: Visit expectations are based on patient acuity, payer requirements, and agency standards; reviewed during onboarding

Mileage/Drive Time: Mileage reimbursement and drive-time practices provided per agency policy

On-Call/Weekends: No on-call or weekend rotation for therapy clinicians; after-hours patient triage available

Primary Responsibilities

Complete in-home occupational therapy evaluations (ADLs/IADLs, functional mobility, home safety, cognition/perception, equipment needs) and establish measurable goals

Develop, implement, and update plans of care consistent with physician orders, payer requirements, and patient-centered goals

Provide skilled OT interventions in the home (ADL training, fall prevention, fine motor skills, energy conservation, cognitive strategies, adaptive equipment recommendations)

Educate patients and caregivers on home programs, safe techniques, and equipment use

Assess home environments and recommend modifications, DME, and community resources

Collaborate with nursing, PT, SLP, physicians, and the interdisciplinary team

Perform reassessments and adjust plans of care as needed

Identify clinical red flags and communicate changes promptly per agency protocol

Complete timely and accurate documentation in Finale EMR

Supervise COTAs and support personnel as permitted by California law and agency policy

Manage field schedule, caseload priorities, and visit coordination

Licensure & Credential Requirements

Active California Occupational Therapist license in good standing

Maintain required continuing education and competencies

Current BLS/CPR certification (or obtained within 30 days)

Qualifications

Degree in Occupational Therapy from an accredited program

Home health experience preferred; strong independent clinical reasoning

Strong documentation and communication skills

Proficiency with mobile documentation systems

Valid California drivers license and reliable transportation

Familiarity with Medicare home health documentation preferred

Finale EMR or similar home health EMR experience preferred

What Success Looks Like (First 90 Days)

Ability to assist with transfers, mobility, and patient positioning
