RN Case Manager | Care Coordination and Utilization Management | Sacramento Region | $48 to $65 an hour

Pure Heart Staffing places this role year-round with community health centers in the Sacramento region; openings start as each client confirms a seat.

Pay: $48 to $65 an hour. This is our good-faith range across the community health centers, health plans and hospital partners we recruit for; the exact figure depends on the client, your experience and any certification.

Where: Sacramento, Yolo and Placer counties; several clients offer hybrid or primarily remote schedules after onboarding.

The work: Complex care management and care transitions for patients with chronic conditions and social needs: assessments, individualized care plans, utilization review and authorizations, discharge planning and coordination with providers, payers and community partners. Enhanced Care Management (ECM) and Medi-Cal managed care experience is especially valued.

What we look for:

• Active California RN license and BLS
• Two years or more of case management, utilization management or care coordination experience; CCM or ACM certification is a plus
• Comfort with InterQual or MCG criteria, EHR and care management platforms
• Work authorization that needs no sponsorship or transfer
