Home Health Admission Registered Nurse (RN) – Full-Time

Position Overview
The Admission RN plays a critical role in initiating patient care by conducting in-home admissions, developing care plans, and ensuring a smooth transition into home health services. This position combines field-based patient care with administrative responsibilities.

Key Responsibilities

• Conduct in-home patient admissions and develop individualized care plans
• Manage and deliver high-quality patient care in collaboration with the care team
• Ensure timely and accurate completion of clinical documentation and charting
• Maintain strong time management and organizational skills to meet deadlines
• Utilize tablets, mobile devices, and EMR systems for documentation and communication

Qualifications

• Associate Degree in Nursing (ADN) required
• Active Registered Nurse (RN) license in the state of Pennsylvania
• Previous patient care experience required; home health experience preferred
• Proficiency with electronic medical records (EMR) and mobile technology
• Strong communication, clinical, and organizational skills

Schedule & Shift Details

• Flexible 4-day workweek (10-hour shifts), with schedule options:
  • Friday – Monday or Saturday – Tuesday
• First 3 days: Field-based admissions
• 4th day: Work-from-home administrative duties
• Flexibility required during the first 6 weeks for training and orientation
