Residential Home Health & Hospice
Required:
- Associate degree or higher
- Must be authorized to work in the United States; visa sponsorship is not available
- 1+ years of experience in ANY of the following: Home Health or Home Health Care
- 1+ years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Practice Partner EMR
- EHR Implementation
- Valid Regular Driver License (DL) in the state of Pennsylvania (PA)
- Valid Physical Therapy Assistant in the state of Pennsylvania (PA)
- Able to travel for this position
Duties
- Assist licensed physical therapists in developing and executing individualized treatment plans for patients of all ages, including pediatrics and geriatrics.
- Administer therapeutic interventions such as trigger point therapy, manual therapy, and modalities to promote healing and mobility.
- Monitor patient progress, document responses to treatments, and communicate observations to supervising therapists for ongoing care adjustments.
- Support discharge planning by preparing patients for transition to home or other care settings through education and self-management strategies.
- Maintain detailed records of treatments provided, patient responses, and progress notes in compliance with healthcare regulations.
- Educate patients on physiotherapy exercises, injury prevention techniques, and proper body mechanics to foster independence.
- Collaborate with multidisciplinary teams in outpatient clinics, inpatient units, or home health visits to ensure comprehensive patient care.
Experience
- Prior experience working as a Physical Therapy Assistant in a Home Health or Home Health Care setting.
- Demonstrated knowledge of anatomy, physiology, medical terminology, and physiotherapy techniques essential for effective patient support.
- Skilled in specialized therapies such as trigger point therapy and orthopedic rehabilitation.
- Experience working within skilled nursing facilities or providing home health services enhances your ability to adapt to diverse patient needs.
- Familiarity with care plans for various populations including orthopedics, geriatrics, pediatrics, and acute care environments is advantageous.
