Qualifications

Required Milford, PA

• Associate degree or higher

• 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Other EMR
  • TruChart EHR

• Valid Licensed Practical Nurse (LPN) in the state of Pennsylvania (PA)

• ANY of the following valid licenses/certifications:
  • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
  • American Heart Association Basic Life Support (AHA BLS)

Our client is seeking a compassionate Home Health LPN to join their extraordinary care team in PA. The ideal candidate will have an associate degree, LPN license in the state of work, and two years of LPN experience. Responsibilities include collaborating to provide patient care plans, supporting patients in the local area, and demonstrating proficiency with technology tools.

Key Responsibilities Collaborate to provide management and delivery of patient care plans Support patients in and around Milford, PA Meet critical deadlines for charting Self-manage time and schedule efficiently Demonstrate experience with tablets, mobile phones, and EMR software 

Schedule and Shift Details Monday – Friday 8:00 am – 6:00 pm, 1 weekend per month, and 2 holidays per year Travel Occasional travel may be required within the local area.
