About the Role

We are seeking a compassionate and experienced Hospice Registered Nurse (RN) to join our dedicated care team in Pittsburgh. This full-time role is ideal for a nurse who is passionate about providing exceptional end-of-life care, thrives in a supportive interdisciplinary environment, and values meaningful patient relationships.

You will play a key role in delivering high-quality hospice services, ensuring comfort, dignity, and support for patients and their families.

Key Responsibilities

  • Provide comprehensive hospice nursing care in accordance with individualized care plans.
  • Conduct patient assessments, monitor symptoms, and manage pain and comfort needs.
  • Collaborate with physicians, social workers, chaplains, and caregivers to ensure holistic care.
  • Document all care activities accurately within the EMR system.
  • Educate patients and families on care processes, medications, and expectations.
  • Participate in interdisciplinary team meetings and care planning.
  • Maintain compliance with all state, federal, and organizational standards.

Required Qualifications

  • Associate degree or higher in Nursing.
  • Active Registered Nurse (RN) license in Pennsylvania (PA).
  • 2+ years of Hospice experience.
  • 1+ year of experience in ANY of the following:
    • Electronic Medical Record (EMR)
    • Other EMR systems
    • TruChart EHR
