What You’ll Be Doing:

• Direct the daily operations of the home health department serving seniors in their homes, nursing homes, and board & care settings
• Oversee nursing, PT, and OT services with a focus on care plan execution and outcomes
• Hire, coach, and support a growing team of home care professionals
• Collaborate closely with the interdisciplinary team (IDT) to optimize care delivery
• Monitor workflow efficiency, compliance, documentation, and participant safety

Ideal Candidate Profile:

• California RN license (required)
• Bachelor’s degree preferred; equivalent experience considered
• 2+ years of nursing experience in home health, elder care, or PACE-like programs
• Strong organizational and leadership capabilities
• Deep commitment to high quality, compassionate elder care
