As a Primary Care Physician (Pod Leader) within a value-based PACE (Program of All-Inclusive Care for the Elderly) model, you’ll deliver holistic, deeply personal care to a smaller panel of medically complex seniors, without hospital rounds or volume pressure.

Your Role:

  • Conduct in-person comprehensive assessments and lead the care planning process
  • Serve as a key clinical leader across the interdisciplinary team (IDT)
  • Coordinate discharge plans and long-term placement as needed
  • Perform home visits as part of care delivery
  • Deliver integrated, preventive, and longitudinal care to medically vulnerable elders

What You Bring:

  • MD or DO with board certification (or eligibility) in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics
  • Valid California medical license (or ability to obtain)
  • 1-2+ years of experience in primary care, home-based care, or geriatric medicine preferred
  • Must be eligible for Medicare/Medi-Cal enrollment
  • Passion for value-based care and supporting underserved aging populations
