What You'll Do:
- Conduct assessments both in facility and occasionally at home to guide care plans
- Participate actively in team-based care planning discussions
- Deliver personalized physical therapy interventions tailored to each participant
- Provide training to caregivers and supervise therapy assistants
- Promote safety, mobility, and independence in a holistic, supportive setting
What You’ll Need:
- Bachelor's degree in Physical Therapy (Master’s or Doctorate preferred)
- Valid California Physical Therapy License (or eligibility)
- 1-2+ years of experience in outpatient, acute, inpatient, or SNF settings preferred
- CPR certification preferred
- Passion for serving frail or elderly populations with compassion and respect
