Home Health Physical Therapy - PRN

Job Description:

Were hiring a compassionate and skilled Licensed Home Care Physical Therapist to deliver high-quality, personalized therapy to patients in their homes. This position is ideal for a PT looking for flexibility, autonomy, and the opportunity to build lasting patient relationships.

Responsibilities:

• Provide in-home physical therapy evaluations and treatments
• Develop and implement personalized treatment plans
• Educate patients and caregivers on mobility safety and exercises
• Document patient progress and update care plans as needed
• Collaborate with interdisciplinary team members
• Ensure compliance with HIPAA, state, and federal guidelines

Requirements:

• Valid Physical Therapy license in the state of Maryland
• Degree in Physical Therapy (DPT, MPT, or BSPT)
• At least 1 year of physical therapy experience (home health preferred)
• CPR Certification
• Valid drivers license and reliable transportation
