Job Summary:

We are seeking a compassionate and skilled Physical Therapist (PT) to join our Home Health Care team. In this role, you will provide rehabilitative services to patients in their homes, helping them regain strength, mobility, and independence following illness, injury, or surgery. You will work closely with patients, families, and other health professionals to develop and implement individualized treatment plans.

Key Responsibilities:

  • Conduct initial evaluations and assess patients physical abilities, mobility limitations, and goals.
  • Develop and implement individualized treatment plans in coordination with physicians and interdisciplinary team members.
  • Deliver hands-on therapy and prescribe therapeutic exercises to improve strength, flexibility, balance, and function.
  • Educate patients and caregivers on exercises, home safety, and use of assistive devices.
  • Monitor and document patient progress, adjusting care plans as needed.
  • Communicate regularly with physicians, nurses, and other care team members to ensure continuity of care.
  • Maintain accurate and timely clinical documentation in accordance with agency and regulatory standards.
  • Ensure compliance with all state and federal regulations and home health care policies.

Qualifications:

  • Doctor of Physical Therapy (DPT) or Masters degree in Physical Therapy from an accredited program.
  • Current state licensure as a Physical Therapist.
  • Minimum of 1 year of experience, preferably in home health or a similar setting.
  • CPR certification required.
  • Valid drivers license and reliable transportation.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to work independently in a home setting.

Preferred Qualifications:

  • Experience with EMR systems (e.g., HomeCare HomeBase, Kinnser, etc.)
  • Bilingual skills a plus.
  • Knowledge of Medicare and home health documentation standards.

Working Conditions:

  • Travel required within service area.
  • May require lifting, transferring, and supporting patients.
  • Flexible hours based on patient needs.
