About the job Housing Stability Program Coordinator
The Member's goal will be to develop and implement a new program that offers wrap-around transportation and case management services for recently housed individuals at high risk of returning to homelessness. The member will work to achieve this goal by carrying out the following duties:
- Create a system for referrals and admission to this program for recently housed individuals and families.
- Create an appointment management system for individuals and families in the program to keep track of medical, mental health, housing, case management, legal appointment dates and times.
- Recruit and coordinate volunteers to offer transportation to and from appointments.
- Develop relationships with partner organizations offering community social events, resource fairs, and food banks.
- Coordinate transportation to and from community social events, resource fairs, and food banks.
- Provide referrals for and coordinate transportation to rental and utility assistance providers when needed
- Participate in organization meetings and decision-making
- Respond to calls and texts on our dispatch phone.
- Provide transportation for the program when volunteers are not available.
