What You'll Do
- Deliver high-quality physical therapy care in patients' homes
- Develop, implement, and manage individualized care plans
- Collaborate with a multidisciplinary care team
- Maintain timely and accurate documentation
- Travel locally to patient residences (~20% of your day)
What We're Looking For
- Bachelor's or Master's degree in Physical Therapy (required)
- Active Physical Therapist license in the state of practice
- Minimum of 1 year of healthcare experience
- Valid driver's license and reliable transportation
- Strong time management and self-scheduling skills
- Comfortable using tablets, smartphones, and EMR systems
- Commitment to meeting documentation deadlines
- A compassionate, patient-centered approach to care
