What You'll Do

  • Deliver high-quality physical therapy care in patients' homes
  • Develop, implement, and manage individualized care plans
  • Collaborate with a multidisciplinary care team
  • Maintain timely and accurate documentation
  • Travel locally to patient residences (~20% of your day)

What We're Looking For

  • Bachelor's or Master's degree in Physical Therapy (required)
  • Active Physical Therapist license in the state of practice
  • Minimum of 1 year of healthcare experience
  • Valid driver's license and reliable transportation
  • Strong time management and self-scheduling skills
  • Comfortable using tablets, smartphones, and EMR systems
  • Commitment to meeting documentation deadlines
