Location: Rural New Mexico
Salary Range: $110,000 - $140,000 (based on experience)
Signing Bonus: Up to $10k

Position Overview

As the Director of Case Management, you will be responsible for overseeing the management and coordination of the entire department. Leading a dedicated team of around 20 healthcare professionals, you will ensure that patients receive the most effective, cost-efficient care throughout their healthcare journey.

You will focus on enhancing both the clinical and financial outcomes by leading the department's strategic direction, collaborating with healthcare teams, and ensuring compliance with industry standards. Additionally, you'll manage the Patient Advocate program, ensuring patients receive exceptional support and advocacy at every stage of their care.

Core Responsibilities:

  • Lead the Case Management department, focusing on smooth patient transitions and high-quality care.

  • Oversee the clinical, financial, quality, and HR functions of the department.

  • Drive initiatives to improve care coordination across various patient care settings.

  • Foster a culture of continuous improvement by educating and supporting your team on best practices.

  • Lead the Patient Advocate program to ensure patients rights are respected and their concerns addressed.

  • Work collaboratively with hospital leadership to maximize care efficiency and outcomes.

Qualifications Were Seeking:

  • Current RN license in New Mexico or a compact license.

  • At least 3 years of experience in Utilization and Case Management.

  • A minimum of 2 years of leadership experience within the healthcare industry.

Additional Preferred Qualifications:

  • Certification from the National Association of Healthcare Professionals or InterQual.

  • Preferred certifications: CCM (Certified Case Manager) or ACM (Accredited Case Manager).
