Columbia Basin Hospital is a 25 bed Critical Access Hospital serving patients across Grant County and the surrounding Columbia Basin region.

The organization provides emergency care, inpatient services, clinic services, and swing bed care, all focused on delivering high quality care close to home.

Patients often rely on one person to help connect services, navigate care plans, and ensure continuity across providers.

In this role, you will:

  • Serve as a central point of contact for patients, families, and care teams
  • Identify unmet medical and social needs
  • Build and manage individualized care plans
  • Support patients through transitions of care
  • Help ensure follow up, medication management, and access to services

Your work directly improves patient outcomes and strengthens the entire care system.

The Opportunity

  • Work four 10 hour shifts per week supporting strong work life balance
  • Partner closely with providers, care teams, and community resources
  • Provide clinical oversight and support to medical assistants
  • Participate in care coordination meetings and quality initiatives
  • Engage in continued learning including health coaching and care coordination development

What You Will Bring

  • Active Washington State RN license
  • Current BLS certification
  • Strong communication and relationship building skills
  • Experience in care coordination or case management preferred
  • Ability to work across teams and support patients holistically
  • Strong organizational skills and attention to detail
  • Valid driver's license and reliable transportation

Compensation and Benefits Highlights
