Role Overview

Our client is seeking a Cardiology Patient Navigator (Coordinator) to support a busy cardiology care team overnight and on weekends. The role centers on patient communication, scheduling, care coordination, remote monitoring alert triage, and clinical documentation. The ideal candidate brings a clinical background, an active nursing license (RN or equivalent), experience in telephonic or remote care settings, and comfort managing a high-volume patient caseload across multiple priorities.

Cardiology-specific experience is preferred but not required. Candidates with strong backgrounds in care coordination, care management, med-surg, ICU, cardiac step-down, CCU, or home health will be competitive. What matters most is clinical credibility, strong communication, and the ability to keep patients connected to their care plan.

Key Responsibilities

Patient Communication & Scheduling

  • Manage patient scheduling, reminders, and follow-up workflows in EMR/EHR systems
  • Conduct telephonic or remote patient outreach to reinforce care instructions and coordinate next steps
  • Support patients navigating cardiology care pathways, including CHF management protocols

Remote Monitoring & Care Coordination

  • Monitor remote patient monitoring alerts for patient weight and blood pressure readings
  • Escalate abnormal readings or clinical concerns to the appropriate provider promptly (no diagnosing or prescribing)
  • Track testing schedules, follow-up appointments, and remote patient monitoring workflows
  • Coordinate with the clinical team to ensure continuity of care across a caseload of 150+ patients

Documentation & Compliance

  • Maintain accurate and timely clinical documentation in EMR/EHR systems
  • Ensure all patient interactions and records meet HIPAA compliance standards
  • Support billing documentation and insurance coordination as needed

Experience & Skills

Required

  • Active nursing license — U.S. state RN license strongly preferred.
  • 3+ years of experience in healthcare, telehealth, or medical office settings
  • Experience in care coordination or care management, preferably in telephonic or remote settings
  • B2+ English proficiency; assessed at screening
  • Experience using EMR/EHR systems and comfort monitoring remote patient monitoring (RPM) alert dashboards
  • Demonstrated AI fluency — actively uses AI tools to work faster and smarter; specific tools and use cases confirmed at screening
  • Strong patient-centered communication skills
  • Able to work the required U.S. Central Time (CT) coverage window — 5:00 PM–8:00 AM CT plus weekends, which falls during Philippine daytime (approx. 6:00 AM–9:00 PM PHT) — starting August 1, 2026
  • Strong written and verbal English communication; assessed at screening

Preferred

  • Experience in cardiology, med-surg, ICU, cardiac step-down, CCU, or home health settings
  • Outpatient cardiology, home health, or telemedicine experience
  • Remote patient monitoring (RPM) exposure
  • Medical billing or prior authorization knowledge
  • Prior experience working as a contractor or in startup/evolving environments
