About the job Medical Social Worker (Community-Based)
Play a vital part in supporting the psychosocial well-being of our residents and their caregivers. You will take ownership of conducting in-depth assessments, coordinating care plans, and providing interventions that promote dignity, comfort, and holistic well-being for our elders.
Responsibilities
- Manage caseloads requiring social work intervention, including conducting BPSS assessments, case management, and coordinating care plans within stipulated timelines.
- Collaborate with multi-disciplinary teams (e.g., through MDMs) to support the psychosocial well-being of elders and caregivers.
- Develop and coordinate discharge and transition plans for elders moving to other services when appropriate.
- Provide community-based case management and make necessary referrals to external agencies.
- Support and co-facilitate therapeutic groupwork/programmes and engage in ACP conversations when suitable.
- Prepare financial assistance reports (e.g., Medifund, Subsidy Deviation) and ensure proper documentation for case files, audits, and reporting.
- Contribute to funded programme statistical reports and participate in quality improvement initiatives/projects.
- Source community support networks and perform other tasks assigned by the clinical supervisor or Head of Department.
Requirements
- Degree or Graduate Diploma in Social Work.
- At least 2 years of direct social work experience.
- Experience working with elders in community or healthcare settings is an advantage.
- Strong interpersonal, organisational, and communication skills; a team player.
- Proficiency in Microsoft Office.
- Able to work independently with good initiative.
