About the role

To facilitate timely admissions, monitor referral trends, and collaborate with various service units to optimise elder placement. The role also involves providing guidance to the team, reviewing policies, and driving process improvements to support operational efficiency and achieve organisational goals.

Key responsibilities

  • Provide leadership to the Referrals & Admissions Unit (RAU) in facilitating timely admissions into SLECs services, monitoring referral numbers weekly, and reporting updates to the Reporting Officers.
  • Assist in training and guiding new and junior staff on the referral and admissions process.
  • Guide the team on the referral process to ensure elders are referred to the appropriate care services and centres.
  • Review relevant policies and procedures and update them when necessary.
  • Monitor overall referral and admission rates and ensure the team meets the organisations targets.
  • Ensure referrals are reviewed and followed up in a timely manner.
  • Identify bottlenecks in the referral process and work with various service unit heads to troubleshoot and facilitate a more efficient workflow.
  • Coordinate with different stakeholders to ensure the team meets required timelines in the referral and admissions process.
  • Track admission turnover rates against required benchmarks and inform relevant service units when new referral or assessment slots need to be opened.
  • Supervise the RAU team in working with various departments and regularly review the referral process to ensure timely admissions.
  • Analyse referral and admission trends and client needs, to facilitate appropriate internal transfers across business units or engage specialised services where required.
  • Serve as the point of contact for stakeholders from other services when coordinating referrals.
  • Support the RAU team in administrative and documentation activities required throughout the referral-to-admission process to ensure a smooth transition for elders into services.

Qualifications & Experience

  • Minimum Diploma, preferably in a healthcare-related discipline.
  • Proficiency in data analytics tools and dashboard solutions.
  • Familiarity with billing processes and client/beneficiary fee management.
  • At least one year of experience in team supervision or management preferred.
  • Experience in referral-to-admission processes within the healthcare or ILTC sector.
  • Strong communication skills with a customer service-oriented mindset.
  • Basic proficiency in Microsoft Excel and PowerPoint.
  • Experience with patient or client management systems would be an added advantage.
  • Ability to work independently and collaboratively as part of a team.
