COMMUNITY CARE EXECUTIVE
We are seeking a passionate and resourceful Community Care Executive to join our Centre-Based Services team. In this role, you will play a vital part in ensuring elders receive quality care and support by monitoring their health conditions, supervising care team, and planning meaningful activities tailored to elders' needs and interests. You will take ownership of overseeing care delivery, supporting the development of Individualised Care Plans (ICP), ensuring accurate documentation, and maintaining a safe and well-functioning centre environment.
Responsibilities
- Supervise and guide Community Care Associates and Senior Community Care Associates in delivering personal and nursing care to seniors.
- Monitor seniors diet, cognition, and behaviour, and respond appropriately to concerns or signs of deterioration.
- Assist with medication administration, including pre-packed and topical medication, and respond effectively in emergencies using CPR, AED, and first aid.
- Support the development and implementation of Individualised Care Plans (ICP) in collaboration with nursing and therapy teams.
- Ensure accurate documentation of seniors assessments, progress, and engagement in activities.
- Maintain environmental safety, infection control standards, and equipment readiness in alignment with organisational policies.
- Plan and lead community activities and festive events, working closely with CCAs and SCCAs.
- Conduct training and orientation for staff, caregivers, interns, and volunteers, while fostering team effectiveness and professionalism.
Requirements
- Minimum Diploma or Degree in a relevant discipline, with at least 2 years of supervisory experience managing teams of 5 or more.
- Prior caregiving experience in hospitals, nursing homes, or home settings is an advantage; career switchers are welcome.
- Patient and willing to serve elderly clients; bilingual in Mandarin is preferred due to the work environment.
- Able to respond professionally to service challenges and escalate issues when necessary.
- Strong team player with good communication, interpersonal, and organisational skills; able to work independently under time constraints.
- Proficient in Microsoft Office applications.
