About the job Senior/Staff Nurse (Home Care)
The Home Care Nurse provides nursing care and services for elders in their homes across Singapore. Home care nurses also support SLEC daycare centres in providing centre-based nursing services for elders within the community.
Key Responsibilities:
- Perform patient assessment: Conduct comprehensive and holistic care assessment and review for clients care needs.
- Design care plans: Develop and regularly review Individualised Care plan (ICP) involving the caregivers / family members (if possible) for clients to meet care needs. Develop Discharge Plan for clients who do not no longer require the service.
- Perform nursing procedures: Perform wound management, stoma care, catheter changes, nasogastric tube changes, injections, blood taking, blood glucose monitoring, oral and tracheostomy suctioning.
- Manage Medication: Medication packing, reconciliation and administration.
- Provide Education: Conduct health education and provide caregiver training (To clients and caregivers, as well as to the community).
- Coordinate care: Provide case management and coordination to link client to required services when necessary. Participate in multi-disciplinary discussions.
- Assist in Healthcare Scheme Applications: Assist with the any relevant application and renewal of government subsides (ie. eSMF, IDAPE, FAR)
- Document and Report Care Rendered: Maintain accurate and timely records of visits, communications, results, care plans, and interventions/procedures.
- Plan and organize daily visit schedules efficiently to manage workload. Provide support for relevant data entry, inventory management (ie consumables), service/nursing report and review of clinical processes.
- Assist Doctors during the home visits if required.
- Support Centre-Based Nursing services with nursing procedures for assigned centres.
- Participate in Nursing and Home-based related projects/programs/initiatives.
- Perform other duties as assigned by Director of Nursing and/or Head of Department
Qualifications & Experience:
- Diploma or Advance Diploma or Degree in Nursing and Registered with Singapore Nursing Board.
- Minimum of three years of experience working in Inpatients or Homecare setting. Geriatric care experience will be an advantage.
- Experience in wound care, dementia care or palliative care will be an advantage.
- Possess strong communication and interpersonal skills to build rapport easily.
- Possess good critical thinking, problem-solving, adaptability, and organisational skills.
- Strong team player and yet able to work independently.
- Able to travel around the community.
