Job Type: Full-time and Part-Time Available
Available Shifts: Mon-Fri 7am-3:30pm or 3pm-11pm ; Sat - Sun 7am-3:30pm or 3pm-11pm (flexible schedules if needed)
Starting Salary : $35/hour

What We Ask:
POSITION SUMMARY:
The LVN provides high-quality clinical service to around 25 program participants living at one of our resident homes located in either Los Angeles, East Los Angeles, or Boyle Heights. A successful LVN collaborates with the treatment team in order to improve client adherence to medications and treatment.
ESSENTIAL FUNCTIONS:

The essential functions include:
- Actively engage clients in services through the development of strong rapport, consistent contact and use of motivational interviewing. Provide outreach services as needed, meeting clients where they are at (service location, clients point of view).
- Complete a medical assessment and intake for all new residents and as needed.
- Prepare and ensure proper completion of the Nursing/Medical Initial Intake documentation including nursing notes, Medication Observation Record (MOR)
- Manage onsite medication administration practices that include medication pre-packing in weekly cassette, administering daily medication, and providing intramuscular injections. Actively communicate medication adherence issues to the treatment team.
- Provide psycho-education to clients regarding medication (i.e. purpose of medication, medication orders) and health issues to increase medication adherence and consistent access to care.
- Ensure clients have access to medications as ordered. Coordinate with pharmacy to obtain medication refills, confirm prescriber orders and problem-solve any issues.
- Coordinate client clinical care with community health partners such as primary/specialty providers as needed.
QUALIFICATIONS:
- Active Vocational Nurse license in the state of California
- Experience working with the homeless, substance abuse, mental health/ with dual diagnosis population preferred
- Bilingual Spanish/English is preferred but not required
- Comfortable working at a residential housing facility
SKILLS AND ABILITIES:
- To perform this job successfully, an individual should have strong knowledge of MS Outlook, MS Excel, MS Word, and Internet software.

- May need to bend, stoop, twist , and pick up heavy materials for short periods of time
