Job Title: LVN/LPN - Licensed Vocational Nurse / Licensed Practical Nurse
Job Description:
We are currently seeking dedicated and compassionate Licensed Vocational Nurses (LVNs) or Licensed Practical Nurses (LPNs) to join our team. Responsibilities:
Administer medications and treatments as prescribed by the physician.
Monitor and record patient vital signs.
Assist with patient assessments and care planning.
Collaborate with the healthcare team to ensure comprehensive patient care.
Provide emotional support to patients and their families.
Maintain accurate and up-to-date patient records.
Adhere to all facility policies and procedures.
Shares information appropriately with member of the multidisciplinary team and other hospital department representatives through verbal reports and written documentation.
Participates in pre-admission evaluation as requested by Program Manager and completes Admission Nursing Assessment on all new Admissions to C/A Residential I & Partial Programs (Including ARCC).
Conducts monthly medication renewals and reviews daily reports.
Responsible for follow up on medical tests and labs. Oversees completion of admission paperwork. Completes medication portion of aftercare forms.
Responsible for tracking medical errors.

Requirements:
Active LVN or LPN license in the state of Vermont.
Psych/Behavioral Health Experience - Required
Experience working with child and adolescent populations - Required

BLS and CPI Certification
