Job Title: LVN/LPN - Licensed Vocational Nurse / Licensed Practical Nurse

Job Description:

We are currently seeking dedicated and compassionate Licensed Vocational Nurses (LVNs) or Licensed Practical Nurses (LPNs) to join our team. Responsibilities:

  • Administer medications and treatments as prescribed by the physician.

  • Monitor and record patient vital signs.

  • Assist with patient assessments and care planning.

  • Collaborate with the healthcare team to ensure comprehensive patient care.

  • Provide emotional support to patients and their families.

  • Maintain accurate and up-to-date patient records.

  • Adhere to all facility policies and procedures.

  • Shares information appropriately with member of the multidisciplinary team and other hospital department representatives through verbal reports and written documentation.

  • Participates in pre-admission evaluation as requested by Program Manager and completes Admission Nursing Assessment on all new Admissions to C/A Residential I & Partial Programs (Including ARCC).

  • Conducts monthly medication renewals and reviews daily reports.

  • Responsible for follow up on medical tests and labs. Oversees completion of admission paperwork. Completes medication portion of aftercare forms.

  • Responsible for tracking medical errors.

Requirements:

  • Active LVN or LPN license in the state of Vermont.

  • Psych/Behavioral Health Experience - Required

  • Experience working with child and adolescent populations - Required

  • COVID vaccination and booster are required.

  • Ability to work effectively in a team-oriented environment.

  • Compassionate and patient-centered approach to care.

  • Attention to detail and strong organizational skills.

  • BLS and CPI Certification
