As a Continence Clinical Nurse with At Home Care, you will work closely with clients experiencing continence issues, getting to know their needs personally and supporting them with dignity and respect. As a registered NDIS service provider, we offer high care services, including private in-home care and nursing, through to social support and daily living assistance. Led by our Nurse Manager and supported by our in-house 24/7 clinical on-call service, our Registered Nurses, Clinical Nurses, and Clinical Nurse Consultants work as a team – supporting each other and advancing in their careers together. About the Role When you’re part of a dedicated nursing team supporting individuals with bladder and bowel dysfunction, every day is an opportunity to make a meaningful impact. In this role you will: Conduct comprehensive continence assessments and develop individualised management plans Deliver evidence-based interventions for bladder and bowel dysfunction Educate clients, carers, and families on continence care strategies and product use Collaborate with multidisciplinary teams including GPs and allied health professionals Support clients to access continence products through funding schemes (e.g., CAPS, NDIS) Promote awareness and education in continence management to health professionals and the community Maintain accurate and timely clinical documentation and reports Contribute to service development and clinical quality improvement Provide mentoring or professional guidance to nursing staff as needed We are currently seeking a passionate, dedicated, and experienced Continence Clinical Nurse to join our growing team. Desired Skills and Experience Postgraduate qualifications in continence or urology nursing Experience with continence product use and management Excellent communication and interpersonal skills Understanding of continence funding schemes (CAPS, NDIS) Proficiency with clinical systems and documentation Knowledge of the disability or community health sector is highly desirable Mandatory Requirements Current AHPRA Registration as a Registered Nurse Demonstrated experience in continence or urology nursing NDIS Worker Screening Check Working with Children Check (WA) Current CPR Certificate Valid driver's licence Right to work in Australia Why Join the At Home Care Team?
