We work closely with participants, their families, and the broader community to deliver person-centred support and connect individuals with the services they need. Position Summary The Support Coordinator is responsible for assisting NDIS participants to understand and implement their NDIS plans, build capacity, and connect with service providers. This role plays a crucial part in promoting choice and control for participants, helping them achieve their goals. Key Responsibilities Assist NDIS participants to navigate and implement their NDIS plan. Coordinate and monitor supports, including mainstream, informal, and funded services. Build participant capacity to manage and maintain their supports independently. Liaise with service providers, allied health professionals, and stakeholders. Ensure services are delivered in line with participants' goals, needs, and NDIS guidelines. Prepare reports and maintain accurate case notes in line with compliance and reporting standards. Respond to crises and provide support in complex situations when required. Skills & Experience Knowledge of the NDIS and relevant legislation. Experience in case management, coordination, or disability services. Strong communication and interpersonal skills. Ability to work independently and manage time effectively. Excellent organisational and problem-solving abilities. Understanding of person-centred and trauma-informed approaches. Qualifications & Requirements Relevant qualification in Social Work, Psychology, Community Services, or similar. Current NDIS Worker Screening Check or willingness to obtain. Valid driver’s licence and reliable vehicle. First Aid Certificate (desirable). Experience with client management systems (desirable).
