About the Client:
Our client is a U.S.-based outpatient eye surgery center specializing in ophthalmic surgical care, including cataract, corneal, and glaucoma procedures. The center's clinical team supports patients through preoperative assessment and surgical care.

Overview:
The Clinical Reviewer reviews preoperative History & Physicals (H&Ps) and related patient records using facility-approved criteria to identify missing, incomplete, inconsistent, abnormal, or clinically significant information requiring follow-up or escalation before the scheduled procedure.

Responsibilities:

1. Philosophy
- Supports the facility's mission, goals, and objectives.
- Performs duties in accordance with facility policies, procedures, and approved H&P review criteria.
- Follows ethical standards and maintains professional conduct.
- Functions as a professional team member and communicates respectfully with facility staff and providers.
- Respects patient rights, privacy, dignity, and confidentiality.

2. Communication
- Communicates clearly, objectively, and professionally with the Clinical Director, charge nurse, anesthesia personnel, surgeons, and designated staff.
- Reports unresolved documentation deficiencies and clinical concerns promptly through the approved workflow.
- Asks for clarification when information is unclear rather than making assumptions.

3. Safety / Risk-Management Program
- Follows facility safety, privacy, documentation, and clinical-review policies.
- Recognizes findings that require escalation according to facility-approved criteria.
- Reports suspected documentation errors, safety concerns, or significant changes in patient condition to the appropriate licensed clinician.
- Does not independently provide medical clearance, diagnose conditions, prescribe treatment, change provider orders, or make final anesthesia/surgical decisions.

4. Professional Competence
- Participates in orientation, competency validation, continuing education, and annual review.
- Maintains current knowledge relevant to preoperative assessment, common medical conditions, medications, and ambulatory ophthalmic surgery.
- Demonstrates consistent use of facility-approved criteria and escalation pathways.
- Accepts coaching and incorporates feedback into future reviews.

5. Duties
- Reviews H&Ps for completeness, date/timeliness, provider signature/authentication, and required elements.
- Verifies patient identity, scheduled procedure, allergies, medications, medical history, and relevant documentation for consistency.
- Reviews pertinent medical history for conditions or changes requiring additional information or escalation.
- Reviews available laboratory results, diagnostic studies, cardiac testing, specialty clearances, and supporting documents when required.

- Maintains an organized work queue and completes reviews within established turnaround expectations.
- Re-reviews charts when additional documentation or corrected information is received.
- Communicates unresolved chart deficiencies promptly and tracks follow-up to completion.
- Uses only approved review criteria and escalates findings outside those criteria rather than independently resolving them.

Qualifications:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN)
- Clinical experience in both ophthalmology and ambulatory surgery; experience in preoperative assessment or clinical chart review is an advantage.
- Proficient in electronic medical records; experience with SIS or Epic is preferred.
- Strong clinical knowledge of medical conditions, medications, allergies, and diagnostic findings relevant to preoperative review.
- Able to review patient records accurately, identify missing or inconsistent information, and escalate concerns according to established guidelines.
