Our client is looking for a Full-Time Home Health Physical Therapist to serve the College Station community. In this role, you aren't just treating an injury; you are helping neighbors in the Brazos Valley reclaim their independence in the environment where they spend 90% of their time.

Key Responsibilities

  • In-Home Evaluations: Conduct comprehensive initial assessments to determine functional limitations and safety risks within the patients actual living space.
  • Functional Mobility Training: Design and lead therapeutic interventions focusing on transfers, gait training, and fall prevention tailored to the patient's home layout.
  • Case Management: Collaborate with a multidisciplinary "pod" including RNs, Occupational Therapists, and Physicians to coordinate the highest level of care.
  • Home Safety Education: Teach patients and families how to utilize assistive devices and modify their environment to prevent re-hospitalization.
  • Electronic Documentation: Maintain accurate, real-time documentation (OASIS) using mobile-based EMR systems.

Qualifications

  • Licensure: Active, unencumbered Physical Therapist (PT) license in the state of Texas.
  • Education: Graduate of an APTA-accredited Physical Therapy program (DPT preferred).
  • Experience: 1+ year of clinical PT experience required; previous home health experience is strongly preferred.
  • Travel: Must have a valid drivers license, reliable transportation, and active auto insurance.
  • Vibe: A self-starter who is comfortable working independently and can navigate the unique social dynamics of home-based care.
