Hospice RN Case Manager

Compensation: $68,000 – $95,000 / Year

Job Summary

We are seeking a Hospice RN Case Manager to lead end-of-life care in the scenic La Grande area. In this role, you arent just a nurse; you are a coordinator, an educator, and a pillar of support. You will manage a local caseload, ensuring every patient receives dignified, expert care in the comfort of their home or residential facility.
Key Responsibilities
- Comprehensive Case Management: Act as the "hub" of the interdisciplinary team, collaborating with physicians, social workers, chaplains, and CNAs to execute a holistic care plan.
- Clinical Excellence: Perform regular physical assessments, manage complex symptoms (pain, respiratory distress, etc.), and administer medications with a focus on comfort and quality of life.
- Family Advocacy: Provide emotional support and essential education to caregivers, helping them navigate the physical and emotional stages of the dying process.
- Patient Admissions: Conduct initial assessments to determine eligibility and assist families in transitioning into hospice services.
- Regulatory Compliance: Maintain meticulous documentation in the EMR (Electronic Medical Record) system to meet Medicare, Medicaid, and State of Oregon hospice regulations.
- Caseload Oversight: Typically manage a localized caseload of 12–15 patients, allowing for high-quality, personalized visits.
Requirements & Qualifications
- Education: Graduate of an accredited School of Nursing (ADN required, BSN preferred).
- Licensure: Active, unencumbered Oregon Registered Nurse (RN) License.
- Experience: * Minimum of 1–2 years of clinical nursing experience (Acute Care, Oncology, or Med-Surg).
- Previous experience in Hospice, Palliative Care, or Home Health is strongly preferred.
- Certifications: Current BLS/CPR certification. CHPN certification is a major plus.
- Soft Skills: High emotional intelligence, exceptional communication skills, and the ability to work independently in a rural field setting.
- Travel: Must possess a valid drivers license, a reliable vehicle, and active auto insurance.
