Human Services Associate/Support Planner needed for our government client in Randallstown. The Human Services Associate/Supports Planner will provide on-going intense case management services to 45 + clients who are older or disabled in a private home or assisted living setting. One year contract position with possible extension or contract to hire consideration. The non-negotiable pay rate is $20.50 per hour.

Essential Duties:

  • Perform in-home assessments to evaluate the health and social needs of participants.
  • Coordinate telephone contact to schedule an in-home assessment.
  • Conduct a face-to-face interview with participant & or responsible party &/or service provider.
  • Client assessments to include review of: general diagnosis and treatment, activities of daily living, and independent activities of daily living.
  • Perform a review of the clients cognitive, functional, and behavior status.
  • Evaluate the appropriateness and effectiveness of Waiver and non-Waiver services.
  • Evaluate and record changes in the participants' general health & well-being.
  • Review medical documentation (i.e., urgent care visits and hospital discharge plans).
  • Complete designated program client assessment forms.
  • Develop a person-centered Plan of Service (POS)
  • Monitor the implementation of services and whether or not a Plan of Service requires a revision.
  • Monitor Community Settings compliance.
  • Assign OTP devices to clients without landline phones for providers to utilize for clocking in when providing services.
  • Check ISAS (In-home Supports Assurance System)
  • Review InterRai assessments, nurse monitor reports, medical. If appropriate, also review adult day care updates, and DDA program reports on a regular basis through Maryland LTSS.
  • Monitor visits by Nurse Monitors for completion in a timely fashion.
  • Complete, monitor and submit reportable events as required.
  • Ensure on-going monitoring of the clients medical condition, monitoring services delivery, advocating on behalf of the clients with the clients physician, insurance companies, and other social service agencies/ programs.
  • Complete required documentation all activities noting the date, time, type of services provided, mode of service and case notes pertaining to entry for Medicaid billable services
  • Conduct a visit that includes a face-to-face interview with participant & or responsible party &/or service provider.
  • Home assessments to include review of: general diagnosis and treatment, activities of daily living, and independent activities of daily living.
  • Review of the clients cognitive, functional, and behavior status.
  • Monitor for indications of abuse and neglect.
  • Evaluate the appropriateness and effectiveness of waiver and non-waiver services.
  • Evaluate and record and changes in the participants general health & well-being.
  • Complete an ALF chart review.
  • Review all 45-day nurse reports ensuring that they are being completed on a routine bases as required and follow-up as needed.
  • Monitor the implementation of services and whether or not a Plan of Service requires a revision
  • Serve as the coordinator of services between the client and the service providers.
  • Monitor Community settings compliance.
  • Review InterRai assessments, nurse monitor, and other medical updates.
  • Review medical adult day care updates, and DDA program reports on a regular basis through Maryland LTSS.
  • Complete, monitor and submit reportable events as required.
  • Review all incidents reports and complete follow-up as needed.
  • Investigate clients complaints of quality of service.
  • Document all case activity in the Maryland Long Term Services and Support electronic web-based program.
  • Complete required documentation all activities noting the date, time, type of services provided, mode of service and case notes
  • pertaining to entry for Medicaid billable services.
  • Perform in-home assessments to evaluate the health and social
  • needs of participants. Visits are to be conducted to meet the requirement of Annual and Quarterly assessments, along with required monthly contacts for each client.
  • Conduct a face-to-face interview with participant & or responsible party &/or service provider.
  • Assess clients ability to reside and participate in programs safely in community setting.
  • Monitor visits by Nurse Monitors for completion in a timely fashion.
  • Assist with financial application and Medicaid re-determination as needed.
  • Record all case activity in the web based, State wide tracking system (LTSS) in a prompt and timely manner.


Qualifications:

  • Must have at least two years of human service case management experience working with the elderly or disabled population.
  • Must have a Bachelor's Degree in Human Services, Psychology, Public Health, Social or Behavioral Science.
  • Ability to learn and utilize Maryland LTSS (Long Term Support Services) is required.
  • Microsoft Word, Excel and Access skills are required.
  • Must be able to accurately assessment client skills.
  • Must be able to learn the operations of Maryland Long Term Services & Supports (LTSS) computer system.
  • Must have good communication skills with the ability to read and interpret regulations.
  • Knowledge of medical conditions/diseases along with terminology is required.
  • Must have own transportation, good driving record and a valid Driver's License with the ability to travel throughout Baltimore County.
  • Must complete CPR and First Aid training.
  • Must be able to work fromk 8:30 am-4:30 pm Monday through Friday with a 1 hour non paid lunch. Client will evaluate a hybrid work schedule after six months of performance.
  • Must be able to pass a background check and drug testing.
