Position Overview:
Tap Healthcare is seeking a dedicated Registered Dietitian to join Welbe Health in Stockton, California. In this role, you'll play a key part in improving the health, wellness, and quality of life of frail and elderly participants through individualized nutrition planning and education. You'll work as part of an interdisciplinary team (IDT) that delivers holistic, person-centered care under the PACE (Program of All-Inclusive Care for the Elderly) model.

Requirements:

  • Registered Dietitian with at least four (4) years of experience directing nutrition services in a similar care setting.

  • Current Basic Life Support (BLS) / First Aid Certification.

  • Reliable means of transportation.

  • Excellent leadership, organization, and communication skills, particularly in settings involving seniors, their families, and interdisciplinary team members.

  • Minimum of one (1) year of experience working with frail or elderly populations.

  • 1-2 years of professional experience required.
