About the job Care Coordinator
As a Remote Care Coordinator, you will be responsible for coordinating care plans, managing patient schedules, and ensuring patients receive the support and services they need in a timely manner. This role requires a detail-oriented, empathetic, and proactive individual who can work independently and collaborate with various teams to ensure a seamless patient experience.
Key Responsibilities:
* Coordinate Care Plans: Develop, implement, and monitor patient care plans in collaboration with healthcare professionals and patients families.
* Patient Scheduling: Schedule appointments, follow-ups, and other necessary medical services, ensuring timely and accurate documentation.
* Patient Advocacy: Act as a liaison between patients and healthcare providers, addressing concerns and ensuring the best possible care is provided.
* Data Management: Maintain accurate patient records, update systems with relevant information, and track progress on care plans.
* Resource Allocation: Identify and provide patients with resources and support, such as community services, home health aides, or transportation.
* Communication: Effectively communicate with patients, families, healthcare teams, and insurance providers to ensure smooth care delivery.
* Compliance: Ensure all patient information and care activities adhere to HIPAA guidelines and company protocols.
Qualifications:
* Experience: At least 2 years of experience in a healthcare, care coordination, or patient services role (preferred).
* Education: A Bachelors degree in Healthcare Administration, Nursing, Social Work, or a related field (preferred).
Skills:
* Strong organizational skills and attention to detail.
* Excellent verbal and written communication skills.
* Ability to multitask and work efficiently in a fast-paced environment.
* Proficiency with electronic health records (EHR) systems and Microsoft Office Suite.
* Compassionate, empathetic, and patient-centered approach.
* Ability to work independently and as part of a remote team.
Certifications:
* Care coordination certification (preferred, but not required).
* CPR/First Aid certification (preferred).
