Job Description:

Responsibilities:
- Schedule: 12 hour shifts;
- Paid Weekly;
- Medical;
- Palliative/Crisis care, supervision of CNA, STNA;
- Contributes to the effectiveness of case management and coordination of services through open communication;
- Communicates changes in patient condition and needed changes to the plan of care to appropriate team members and caregivers;
- Participates in staff and IDT meetings as requested;
- Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company;
- Prepares clinical and progress notes on the relevant and specialized nursing services he/she provides;
- Attends nursing home case conferences as requested;
- Assists the RN and/or physician in performing specialized duties related to end-of-life care;
- Follows directives of case manager and supports the efforts of case coordination;
- Assists the patient in learning appropriate self-care techniques;
- Delivers care to assigned clients and performs direct care in accordance with an established plan of care;
- Administers medications and observes patients for adverse reactions to medications or treatments.
Requirements:
- LPN License;
- Clean Background Check and Clean Drug Screen;
- Must have access to reliable transportation;
- Current CPR, TB, Physical;
- Assists the patient in learning appropriate self-care techniques;
- Ensures the maintenance and confidentiality of client records and employee information in accordance with Confidentiality policy;
- Follows directives of case manager and supports the efforts of case coordination;
- Assists the RN and/or physician in performing specialized duties related to end-of-life care;
- Attends nursing home case conferences as requested;
- Prepares clinical and progress notes on the relevant and specialized nursing services he/she provides;
- Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company;
- Contributes to the effectiveness of case management and coordination of services through open communication;

- Prepares equipment and materials for treatment adhering to aseptic technique as required;
- Communicates changes in patient condition and needed changes to the plan of care to appropriate team members and caregivers.
