About the Role:
Join a respected healthcare organization serving our military community as a Remote RN Disease Manager. In this full-time, temp-to-hire role, you will provide condition-specific education, coaching, and care coordination for TRICARE beneficiaries across the U.S.

This is an excellent opportunity for experienced RNs with a background in chronic disease management, care coordination, or case managementparticularly those located in compact states or willing to work MST/PST hours. We are especially seeking candidates in California with behavioral health or pulmonary experience.

Schedule & Location Requirements:

• Full-Time: 40 hours/week

• Shifts: Must be available to work MST/PST or later

• Remote: Must reside in one of the approved work states (listed below)

• High Priority States: CA, AZ, NV, CO, WA, OR, TX, UT, NM

Preference for candidates residing in or willing to work:

• California (1 RN with pulmonary experience, 1 RN with behavioral health experience)

• Compact states or CST candidates willing to work late shifts

Key Responsibilities:

• Provide disease management services to TRICARE beneficiaries

• Collaborate with care teams to assess, plan, implement, and evaluate individualized care plans

• Empower patients to manage chronic health conditions through coaching and education

• Monitor outcomes and adjust care plans accordingly

• Maintain accurate, timely documentation

Required Qualifications:

• Current, unrestricted RN license in state of residence

• U.S. Citizen

• Able to obtain favorable DoD background investigation

• Able to obtain multi-state licensure if in a Nurse Licensure Compact (NLC) state

• At least 3 years of clinical experience

• Background in case management, care coordination, or discharge planning in acute/ambulatory/specialty settings within a healthcare system or MCO

Preferred Qualifications:

• Bachelors degree in nursing or related health field

• Experience in disease management within a Managed Care Organization (MCO)

• Familiarity with TRICARE or government healthcare programs (Medicare/Medicaid)
