Salary Range: $114,296.00 $148,584.80 annually

Minimum Experience: Two years in a clinical health setting preferred; new graduates with relevant license may be considered
License: Active Alaska RN license required (or eligibility upon acceptance)
Certifications: Must obtain CDCES (Certified Diabetes Care and Education Specialist) within 2 years; BLS preferred
COVID-19 Vaccination: Proof of completed COVID vaccinations required
Holiday Coverage: Not required
Bilingual: Yupik/English preferred

Position Summary

The Registered Nurse Case Manager/Clinical Diabetes Educator is responsible for providing comprehensive diabetes education and care for patients with Type 1, Type 2, gestational diabetes, and those at high risk of developing diabetes in a clinic setting. Education is delivered via telephone, video teleconferencing, and in-person sessions, including travel to villages as needed. The educator orders labs, documents education in the electronic health record, and assists in planning and coordinating diabetes-related health promotion activities throughout the region.

This role collaborates closely with village clinic and hospital staff to promote diabetes standards of care, inform about advances in diabetes management, and maintain the Diabetes Registry and grant requirements. The educator participates in outreach activities such as support groups, media campaigns, and community events, and works with the department to achieve excellence in patient care and partnership with tribal leaders.

Key Responsibilities

  • Provide diabetes education and care to patients in clinic, via telehealth, and during village visits.
  • Order labs and document patient education in the electronic health record.
  • Plan and coordinate diabetes-related health promotion activities and outreach.
  • Collaborate with village clinics, hospital staff, dietitians, and pharmacists for comprehensive care.
  • Maintain the Diabetes Registry and assist with grant reporting requirements.
  • Participate in community outreach, support groups, radio shows, and special events.
  • Stay current on diabetes care standards and advances in diabetes management.
  • Promote organizational mission and cultural values in all interactions.
  • Perform other duties as assigned.

Qualifications

  • Completion of a nursing program from an NLN-approved institution.
  • Minimum Associate's Degree in Nursing required; BSN or MSN preferred.
  • Active RN license in Alaska, in good standing.
  • Two years of clinical health setting experience preferred; new graduates may be considered.
  • Must obtain CDCES certification within two years of hire.
  • BLS certification preferred.
  • Proficiency with PC computers and medical equipment related to diabetes care.
  • Ability to travel to villages via small plane and work in sub-arctic conditions.
  • Strong knowledge of diabetes, cardiovascular disease, and public health.
  • Excellent cross-cultural communication skills; bilingual (Yupik/English) preferred.
  • Ability to collaborate with diverse disciplines and community partners.
