Job Title: Licensed Practical Nurse (LPN) Day Shift
Location: Flagstaff, AZ
Position Status: Full-Time, Contract
Schedule: 12-hour Day Shifts | 6:00 AM 6:00 PM | Every Other Weekend
Hours per Week: 3648 hours
On-Site Required: Yes (Local Candidates Only)
Position Overview:
This role involves administering medications, providing direct patient care, supporting treatment plans, and contributing to the overall wellness and comfort of our veteran residents.
Key Responsibilities:
- Administer medications and perform physician-ordered treatments within LPN scope of practice
- Observe and report the effectiveness of medications and treatments
- Communicate with interdisciplinary teams, residents, and families regarding patient care
- Conduct initial admission assessments and support MDS and care plan development
- Maintain accurate clinical documentation and medication records
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
- Assist with shift reports, supply checks, and resident condition monitoring
- Comply with state and federal regulations regarding patient care and medication control
Knowledge, Skills & Abilities:
Knowledge of:
- Geriatric nursing theory and patient care
- Medication administration protocols and gerontology pharmacology
- Team nursing concepts and interdisciplinary coordination
Skills in:
- Performing treatments and medication administration
- Monitoring patient behavior and progress
- Communication and patient documentation
- Understanding and applying applicable laws and regulations
Ability to:
- Assess, evaluate, and respond appropriately to medical emergencies
- Communicate effectively with patients, families, and medical staff
- Maintain clinical professionalism and compassionate care delivery
- Work efficiently as part of a team in a fast-paced healthcare setting
Required Qualifications:
- Active and unrestricted Arizona LPN License
- Current CPR/BLS Certification
- Level 1 Fingerprint Clearance Card
- 2-Step TB Test
- Hepatitis B vaccination or declination
- COVID vaccination or declination
- Minimum education: Associates Degree
- Ability to commit to a full-time day shift schedule
Candidate Requirements:
- Must be a local AZ resident at the time of submission
- Must be available for in-person interview within 1 week of posting close
- Must be able to start within 2 weeks of offer
