Community Mentor

Job Summary:

This position will assist the Community Service Manager with all day-to-day operations of the Adult Day Training program.

Summary of Essential Job Functions:

• Responsible for creating and leading person-centered and interactive activities both onsite and in community settings for participants who attend our ADT Program
• Responsible for required ADT documentation and monitoring as designated by the Community Services Manager
• Responsible for ensuring that ADT activities promote independence and growth per the participants chosen goals
• Assist in participant personal care needs, such as assistance with eating and personal hygiene
• Participate in required training sessions and morning meetings as designated by the Community Service Manager
• Other duties as assigned

Minimum Requirements:

• Must be at least 18 years of age
• High School Diploma, or GED
• Personal Vehicle, Valid Drivers License and Current Auto Insurance
• Physically able to assist in lifting people up to 250 pounds, as well as able to assist in the mobility of wheelchairs containing people up to 250 pounds

Abilities Required:

• Must show an ability to demonstrate patience and respect for those we serve as well as co-workers and management staff
• Ability to provide accurate and timely documentation
• Ability to maintain and demonstrate adherence to all driving safety requirements as outlined by the State of Kentucky
• Must possess the ability to work well with minimum supervision
• Creativity
• Great Communication and Organization Skills, Self-motivated, and the ability to work well in a team environment
