P-Physical Therapist

Position: Physical Therapist

Salary Range: $39-45/hour

Schedule: 35 hours per week, Monday-Friday, no weekend rotation

Facility: Skilled Nursing Facility (SNF) with short term and long term rehabilitation services, as well as a 17 bed assisted living facility (ALF)

Services: Offering occupational therapy (OT), physical therapy (PT), and speech language pathology (SLP) services to patients in both the SNF and ALF settings.

Responsibilities:

- Conduct thorough evaluations of patients to determine their physical therapy needs and develop individualized treatment plans.

- Provide hands-on therapy to patients, including exercises, manual therapy, and other modalities as needed.

- Monitor and document patient progress, adjusting treatment plans as necessary.

- Communicate with other healthcare professionals, including occupational therapists and speech language pathologists, to coordinate patient care.

- Educate patients and their families on home exercise programs and techniques to improve overall function and mobility.

- Maintain accurate and timely documentation of patient progress and treatment plans.

- Collaborate with interdisciplinary team members to ensure comprehensive and effective patient care.

Qualifications:

- Must hold a valid Physical Therapy license in the state of [state].

- Minimum of 1 year experience as a Physical Therapist in a SNF setting preferred.

- Strong communication and interpersonal skills.

- Ability to work independently as well as part of a team.

- Excellent time management and organizational skills.

- Experience with electronic medical records preferred.
