Position Overview

Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a Home Health assignment with Mercy Home Health in Sacramento, CA.

This role involves delivering in-home physical therapy services, focusing on patient recovery, mobility improvement, and independence in a real-world environment.

Clinical Responsibilities

• Perform in-home patient evaluations and assessments
• Develop and implement personalized treatment plans
• Provide gait training, mobility, and strength rehabilitation
• Assess home environment for safety and functional needs
• Educate patients and caregivers
• Monitor progress and communicate with physicians
• Maintain accurate documentation using HCHB EMR

Required Qualifications

Active California PT License
BLS Certification
OASIS Experience REQUIRED
Minimum 2 years healthcare experience

Strong documentation and communication skills

Scheduling & Assignment Details

• Schedule: 5x8 Days
• Hours: 40 hours/week
• Weekend: Every other weekend (EOW)
• On-call: Not specified

Important Notes

Must have OASIS + Home Health experience
Requires 2 supervisor references at submission
Mileage must be tracked and approved
Cannot have worked with CommonSpirit in past 1 year
